Why Momentum

At a lot of clinics, your “PT session” is ten minutes with a therapist and thirty-five on a machine while they see three other patients. Momentum was founded in 2015 to be the opposite: one patient, one Doctor of Physical Therapy, 45 full minutes.

We also believe rehab should prove it’s working. Every four weeks we retest the same objective measures from your first evaluation — strength, range, function scores — and go over the numbers with you. If the data isn’t moving, the plan changes.

    Book your evaluation

    No referral needed. Call or book online and you’ll typically be evaluated within 48 hours — history, movement exam, and baseline measurements.

    Train with your PT

    A plan built around your goal — lift your kid, run the Mini, get back on the court — delivered in 45-minute one-on-one sessions with the same therapist.

    Retest and graduate

    Every 4 weeks we re-measure against your baseline. When you hit your numbers, you graduate with a maintenance program — we don’t stretch out visits.

Before your first session.

Do I really not need a doctor’s referral?

Correct — Indiana allows direct access to physical therapy, so you can start with an evaluation right away. Some insurance plans still require a referral for coverage; we check yours when you book and handle the paperwork if one is needed.

What does PT cost with insurance? Without?

We’re in-network with most major plans — typical copays run $20–$50 per visit depending on your plan, and we verify your exact benefit before your evaluation. Self-pay is a flat $120 per session, evaluation included, with package pricing available.

How many visits will I need?

Most orthopedic cases run 8–12 visits over 6–10 weeks; post-surgical protocols run longer and follow your surgeon’s timeline. You’ll get an estimated visit count at your evaluation, and the 4-week retests keep it honest.

Is it really one-on-one the whole time?

Yes — 45 minutes with your Doctor of Physical Therapy, every visit. No aides running your exercises, no double-booked slots, and you keep the same therapist for your entire plan of care.

Does dry needling hurt, and is it covered?

Most patients feel a brief muscle twitch and some post-session soreness, similar to a hard workout. Coverage varies by plan — when it isn’t covered, it’s a $40 add-on to your session. We’ll always tell you before it’s on the bill.
